- Make sure that the Phaco Tip is tight.
- Check that the Phaco Tip is not bent.
- Check for any damage to the mouth of the Phaco Tip.

| Solution |
- Over used Sleeves can cause poor follow ability because they allow too much flow out the front of the sleeve.
- Over used Sleeves can also cause heating in the incision because internal ribs are worn down.
